I've been going back and forth with several people on YouTube about 7-OH. The argument is the one you'll hear all over Reddit. The claim is that 7-OH and kratom products have a "ceiling effect," so they can't shut down your breathing, and that nobody has ever died from 7-OH or kratom use by itself. They believes the government is banning a safe plant for no reason.


I understand why people believe this. It sounds reassuring, and it's what the vendors want you to think. But a new study just measured what these drugs do to breathing, and the numbers don't support the safe story. Let me show you what the research found and why it matters for your life.



What the New Study Actually Measured


In April 2026, researchers published a study in the Journal of Pharmacology and Experimental Therapeutics. They wanted a simple answer to a serious question. What do kratom's two main compounds do to breathing, and does Narcan reverse it?


They gave rats three drugs, one at a time, straight into a vein. The first was mitragynine, the main alkaloid in the kratom leaf. The second was 7-hydroxymitragynine, better known as 7-OH. The third was morphine, used as the yardstick because every doctor knows how morphine affects breathing.


Then they watched the animals breathe in a sealed chamber that records every breath. They tracked how fast the rats breathed, how deep each breath was and how much air moved through the lungs each minute. That last number, minute volume, is the one that keeps you alive. Drop it far enough and you die.


I want to explain a term first. "Respiratory depression" is the medical phrase for breathing that slows down and gets shallow. It's how opioids kill. Your body forgets to breathe, oxygen drops, and if no one steps in, your heart stops. That's the whole danger in one sentence.



The Two Kratom Alkaloids Do Opposite Things to Your Breathing

Here's the part that surprised even the researchers. The two compounds in the same plant pushed breathing in opposite directions.


Mitragynine, the main ingredient in plain leaf kratom, did not act like an opioid. It sped breathing up. The rats breathed faster, not slower.


7-OH did the opposite. It slowed breathing down. Fewer breaths, shallower breaths, less air per minute. It looked exactly like morphine on the monitor.


So when someone tells you "kratom is an opioid that stops your breathing," they're half right. It depends on which compound you're talking about. The leaf's main alkaloid has a gentle breathing profile. The concentrated 7-OH in gas station gummies and shots is the dangerous one.


There's a reason for this, and it's worth knowing. Classic opioids like morphine and fentanyl shut down breathing partly through a second switch inside the cell, a protein called beta-arrestin. Mitragynine barely touches that receptor. 7-OH flips it hard, the way a full opioid does. Same plant, two very different drugs.



7-OH Is Worse Than Morphine, and There Was No Ceiling


This is where the "ceiling effect" argument falls apart. In the study, 7-OH was 4.5 times more potent than morphine at cutting minute volume in half. Read that again. It took less than a quarter of the drug to do the same damage to breathing that morphine does. This fits older research showing 7-OH is somewhere between oxycodone and fentanyl in strength.


A ceiling effect means the drug hits a limit. Past a certain dose, more drug doesn't slow your breathing any further. Buprenorphine, the medicine in Suboxone, works this way, and that ceiling is a big part of why it's so much safer than other opioids. People hoped 7-OH had the same brake.


The breathing data doesn't show that brake. As the dose went up, breathing went down, in a straight line, just like morphine. If there were a real protective ceiling at doses people actually use, you'd expect breathing to level off. It didn't. So the claim that 7-OH "can't" depress your breathing is wrong. It depresses breathing more than morphine, and it keeps going as the dose climbs.



"But Nobody Dies From 7-OH Alone"


This is the second half of the YouTube argument, and I want to be fair about it. It's true that clean, proven cases of death from 7-OH and nothing else are hard to find in the medical literature right now. But they do exist. However, this is a weak thing to lean on, for three reasons.


First, these 7-OH products are new. Concentrated 7-OH tablets and shots showed up in stores in the last couple of years. The concentrations also vary. Most medical examiners don't even test for 7-OH, so a death can get blamed on "an unknown opioid" or missed entirely. However, the Florida medical examiner reported several cases of deaths from kratom based products with NO OTHER substances found in the toxicology tests.


Second, the pharmacology says death is expected. A drug that stops breathing worse than morphine, with no ceiling, and that responds to Narcan, is a drug that can kill you on its own. We don't need to wait for a pile of bodies to know that. We know how opioids work.


Third, the real-world fatal overdose reports are already here beyond the Florida report. Health departments in Pennsylvania, Texas and Los Angeles County have flagged serious 7-OH cases. In one Pennsylvania report covering 2022 to mid-2025, 14 people needed a ventilator to breathe for them and 25 got naloxone. Los Angeles County put out an alert about deadly overdoses tied to concentrated 7-OH. Alcohol was often in the mix, which brings me to the real danger.



Why Mixing Is the Thing That Kills


Almost nobody with an opioid use disorder takes just one drug. That's where people die. 7-OH slows your breathing. So does alcohol. So do Xanax and Valium. So do other opioids. Stack two or three of these together and the effect isn't added, it's multiplied. An accidental overdose is just waiting to happen. Your brain's drive to breathe gets switched off from several directions at once.


I've written before about how 7-OH follows the path of heroin, and the mixing problem is central to it. The Florida mortality data on kratom told the same story. Most people who died with kratom in their system had other drugs on board too, and a large share had fentanyl. A "safe herbal supplement" that you take with a few beers is not safe at all.



The Leaf Isn't Equally Safe for Everyone


Here's a point most articles miss, and it's why I can't just tell you plain leaf kratom is fine. Your liver turns mitragynine into 7-OH using an enzyme called CYP3A4. You make some 7-OH every time you use kratom, even from plain leaf. The catch is that people run this enzyme at different speeds, based on their genes.


If you're a fast metabolizer, your liver converts mitragynine into 7-OH quickly and in larger amounts. You make more of the dangerous compound from the same dose of leaf. A slow metabolizer makes less. So two people can take the identical product and have very different drugs floating in their blood.


This is why one person swears kratom is harmless and another gets hooked fast or ends up in the ER. It isn't willpower. It's biology you can't see. The plant that's mild for your friend can convert into something much stronger inside you.



We've Seen This Movie Before


None of this is new. It's the opium story all over again. For centuries, people used raw opium and called it natural medicine. Then in the 1800s, a chemist pulled morphine out of the poppy, a concentrated form many times stronger. Doctors called morphine safer than opium. Soldiers came home addicted by the thousands.


Then in 1898, the Bayer company sold heroin as a "non-addictive" cough remedy and a safer version of morphine. That went about as well as you'd guess. Each step, someone took a natural plant, concentrated its strongest opioid, and sold it as the safe upgrade. Each step made addiction worse.


7-OH is the same move with a new name. Take the tiny alkaloid that's 0.01 percent of the leaf, concentrate it into a tablet, and sell it in a gas station as wellness. A 2025 review in Pharmaceutical Biology laid this out plainly and called concentrated 7-OH a public-health threat. History is repeating itself.



The One Piece of Good News


There's a bright spot in the study, and it could save a life. Naloxone, the drug in Narcan, fully reversed the breathing damage from 7-OH. That confirms 7-OH works through the same opioid receptors as heroin and fentanyl, so the same rescue drug works on it. If you or someone you love uses 7-OH, keep Narcan in the house. Learn the signs of an overdose. Slow or stopped breathing, blue lips, pinpoint pupils or someone you can't wake up. Give the Narcan and call 911. Interestingly, the same naloxone did nothing to mitragynine's faster breathing, which is more proof that the leaf and the concentrate are different animals.


Narcan is a backup. The goal is to not need it.

If you're using 7-OH and telling yourself it's safe because it's plant-based, please rethink that. The breathing data puts it below morphine on the safety scale. If you're using plain leaf kratom, understand that your own liver may be turning it into a much stronger drug, and you have no way to know how much.


Don't mix it with alcohol, benzos or other opioids. Keep Narcan nearby. And if you can't stop on your own, that's not a character flaw, it's what these drugs are built to do. Find an addiction doctor who understands kratom and the treatment for it.

On breathing, the leaf's main compound looks safer, since it sped breathing up instead of slowing it down. But plain kratom isn't risk-free. Your liver converts it into 7-OH, and fast metabolizers make more of it, so the same product can be much stronger in one person than another. Furthermore, seizures can occur with kratom products too.
